CareSign is clinical documentation for home health, hospice, private duty home care and mobile wound care practices. Sign-in requires a CareSign account from your practice or agency. Practices contract with CareSign directly; to set up your practice or book a demo, visit caresignhealth.com/contact. This app is for clinicians, aides, caregivers and office staff. Patients, please contact your care provider. CHART AT THE POINT OF CARE • My Day lists today's visits or shifts in order, with directions. • Clock in and out at the patient's home. CareSign records the time and compares your location with the patient's address. • Write and sign visit notes for nursing, therapy, social work and aide visits. • Record each aide task as done, refused or not applicable. • Keep working without a signal. Visits and notes wait on the phone and send when you are back online. WOUND CARE • Wound assessments with photos taken inside the app. Photos are not saved to your photo library. • Trace to Measure: trace the wound edge on a calibrated photo to get length and width, then confirm against a ruler measurement. • Wound care plans you sign on the phone, and a body map showing where each wound is. • Talk the visit: describe the visit out loud and CareSign drafts the wound note for you to review. Speech becomes text on your phone. After you agree, the text goes to CareSign's AI drafting service. Your practice decides whether to turn it on. SIGNATURES • Patients and caregivers sign consents and forms on your phone. • Each signature keeps a record of who signed and when. • Acknowledge signed physician orders assigned to you. TEAM AND OFFICE • Message your care team, with photos and attachments. • Communication notes, and infection and incident reports. • Mileage between visit stops. • Ask CareSign answers how-to questions from the CareSign guide. PRIVACY • Face ID unlock. • Delete my account, in Settings. CareSign is documentation software. It does not diagnose or recommend treatment.
