MiPHR addresses the need to improve communications between healthcare providers (HCPs) and their patients by leveraging mobile technology to enhance the HCP-patient partnership. The app provides a method to monitor and change habits that may improve health outcomes. The MiPHR mobile app has reporting capabilities to inform the HCP. The HCP can utilize this supplemental information as a source for patient management. Our mission is to deliver a platform where individuals can share their personal health information with their healthcare providers to improve outcomes and reduce the cost of care. Our vision is to connect healthcare providers and patients to manage physiological conditions remotely, anywhere, anytime. Our purpose is to give individuals control of their healthcare information, allowing them to partner with providers to live healthier lives. MiPHR has developed a mobile application to address the rising cost of chronic condition management including diabetes, hypertension, and obesity. The mobile application is designed to manage patients diagnosed with chronic conditions, connecting them with their participating providers to received recurring reports for remote physiological management and chronic care management. Subscription Plans (Yearly, Auto-Renewable): Premium Plan: Unlock all features, including sending reports to your selected healthcare provider. ($12.99 per year) Standard Plan: Access all features except report sharing with participating healthcare providers. ($2.99 per year) Payment and Renewal Information: Payment will be charged to your Apple ID account at the confirmation of purchase. Subscription automatically renews unless it is canceled at least 24 hours before the end of the current period.
